The 3-Step Family Action Plan
Print this section and keep it in the front of the binder — it is what the family reads first.
Step 1 — Contact the Funeral Director
Provide the DD-214 (Member-4 Copy) and VA Form 27-2008 for the Burial Flag. The funeral home coordinates the flag request with the VA on your behalf. This is time-sensitive and must happen before burial — it cannot be done after interment.
Step 2 — Call the VA at 1-800-827-1000
Report the veteran's passing immediately to stop ongoing disability payments. Overpayments must be repaid by the estate. In the same call, initiate the DIC claim (VA Form 21P-534EZ) and Burial Allowance claim (VA Form 21P-530EZ). Keep a record of the date, time, and representative name.
Step 3 — Request the Presidential Memorial Certificate
Mail or fax VA Form 40-0247 to your nearest VA Regional Office. There is no deadline. Multiple copies can be requested — one for each immediate family member. This signed acknowledgment of the veteran's service is a meaningful keepsake families often overlook.
Specific Form Checklist

VA Form 21P-530EZ — Burial Allowance
Required documents: DD-214 (Member-4) + Death Certificate + Receipts for private burial costs.
Deadline: 2 years from date of death for non-service-connected deaths. No time limit for service-connected deaths.
Amounts: $2,000 for service-connected death; $1,002 burial + $1,002 plot allowance for non-SC enrolled veterans (FY 2026).
VA Form 27-2008 — Burial Flag
Required documents: Service dates + Branch of Service.
Deadline: Must be coordinated with the funeral director before burial — coordinate this at the time of death, not after.
Note: The funeral home handles submission directly. One flag per veteran — additional flags are not issued.
VA Form 40-0247 — Presidential Memorial Certificate
Required documents: Full legal name of the veteran (no rank needed).
Deadline: No deadline — request anytime after the veteran's death.
Note: Mail or fax to the nearest VA Regional Office. Multiple copies are available for immediate family members.
VA Form 21P-534EZ — DIC (Dependency and Indemnity Compensation)
Required documents: Marriage Certificate + proof of service-connected death OR documentation of 100% P&T status for 10+ continuous years before death.
Deadline: No strict deadline, but file immediately — monthly payments of $1,699.36 (2026 rate) begin from the date of claim, not the date of death.
Eligibility: Surviving spouse of a veteran who died from a service-connected condition, OR who was rated 100% P&T for 10+ continuous years before death (5 years from discharge for certain cases).
All forms require the Member-4 copy of the DD-214. Keep 2–3 certified copies stored separately — one with this binder, one in a fireproof safe, one with a trusted family member.

What is the most urgent task immediately after a veteran dies?
Call VA at 1-800-827-1000 to stop ongoing disability payments within 24–48 hours. If the VA continues paying after the veteran's death, the estate owes the money back. Then contact the funeral director about the Burial Flag (VA Form 27-2008) before the burial — this cannot be done after interment.
What is DIC and who qualifies?

Does the veteran's disability rating affect burial benefits?

What if the veteran has no DD-214 on file?
Request a copy from the National Personnel Records Center (NPRC) using SF-180 or online at archives.gov. Allow 10–30 days standard; expedited service is available for active VA claims. VSOs can also assist with records reconstruction. Keep 2–3 certified Member-4 copies once obtained.
Is the Presidential Memorial Certificate automatic?
No — it must be requested using VA Form 40-0247. There is no deadline and the form only requires the veteran's full legal name. Families can request multiple copies for children and siblings. It is a signed acknowledgment of the veteran's service from the President, and families often overlook it in the rush of early bereavement.
